PRIVACY POLICY AND CONSENT TO SHARE INFORMATION
As a healthcare provider in the private sector, EyeHub is bound by the National Privacy Principles outlined in the Privacy Act 1988. These govern how we collect, handle, use, distribute and store personal information collected from our patients at our clinics.
When dealing with other healthcare professionals, we will be required to disclose personal medical details to obtain an accurate diagnosis or treatment options. By signing this form, you agree and consent to these details being shared when you need them.
DISPARAGEMENT CLAUSE AGREEMENT
The Parties agree not to make any harmful, defamatory, or disparaging statements or remarks, whether oral, written, or electronic, about the other Party, including the Medical Practice, its staff, services, or business operations, to any third party.
This restriction does not apply to truthful statements required by law or, as part of legal or regulatory proceedings. In the event of a breach, the non-breaching Party may seek remedies, including injunctive relief and damages.
PAYMENT AGREEMENT
EyeHub offers consultations, diagnostic tests, and surgical care to our patients. You’ll need to settle all consultation and treatment fees on the day you come. All accounts are finalised before you leave.
Before you schedule your procedure, we will send you a written quote detailing all your expected costs. A deposit secures your surgery date. You are required to pay the remaining balance one week before your surgery. Unpaid balances may delay or result in the cancellation of your procedure.
We wish to highlight that seeking treatment without the intention of paying is unlawful. It is a criminal offence to obtain services through deception. Attending appointments or agreeing to surgery while planning to avoid payment falls under this offence.
EyeHub uses respectful and lawful steps to manage unpaid accounts. These steps include reminders, structured payment discussions, and external recovery support when needed. Our goal is to ensure that your care is seamless and that our services are accessible and equitable for all patients.
