Your journey at EyeHub
We endeavour to make your journey through EyeHub as seamless as possible. Here are a few helpful tips.
Where to find us
Our practice is in building A, on the second floor of the Nucleus Medical Suites, next to Buderim Private Hospital. There is plenty of parking available on site. Please take the lift in the blue block to the second floor, turn left upon exiting and head to the end of the corridor. We are the last suite on the right and clear signage will guide you through. The lifts are wheelchair accessible if needed.
For families: Toilets and baby changing facilities are available on the same floor, opposite the lifts. EyeHub has some toys and books to entertain your little ones. We usually put on cartoons during our paediatric days. Please take caution with prams and strollers in the practice as space is limited.
Pre-arrival
A new patient registration form is available online and we do advise completion ahead of your first visit. Please complete the form below.

We aim to streamline your visit as much as possible. Keep in mind this involves visual screening, tests, and consultation. Please therefore allocate up to two hours for your appointment and bring along your most recent pair of glasses.
You will first be registered by our friendly receptionist, then assessed by our engaging ophthalmic technician before meeting Dr Moorthy. Dilating drops may be required for a comprehensive assessment on the first visit. This may blur your vision and make you very sensitive to sunlight. It is therefore advisable to bring sunglasses and have a companion drive you.
Subsequent visits tend to be more concise, and you will be advised of your follow-up appointment prior to leaving EyeHub.

Health funds
We work with a few health funds as known gap providers for specific surgeries. Please enquire at the time of booking your surgery which health funds we work with.
Please ensure you check with your health fund about your cover prior to your appointment or confirming your surgery. Please bring your health fund membership card with you, as well as your most current Medicare card, so we may ensure the details are correct. Incorrect details may lead to a delay in your Medicare or Healthfund rebates and refunds.
Fees
Adult consultations
Fees are payable at the time of consultation by EFTPOS, bank debit card, Mastercard, Visa, or American Express. Our receptionist will advise you of the estimated charges when you book an appointment with us.
Paediatric consultations
We have very limited paediatric slots. Usually there is more demand than there are appointments. Therefore, we do take a $100.00 NON-REFUNDABLE DEPOSIT for all new paediatric appointments. This will be deducted from the final cost of the consultation.
We do allow one change of date for new paediatric appointments, so long as you give us 10 working days’ notice, or if the child is ill, within 48 hours of the appointment. Please note that a sick certificate will be required. If you do not meet these criteria, then the fee is not refunded.
Please note that consultations and their associated fees are not covered by your health fund, but you may receive a Medicare rebate.
Surgical bookings
We want you to be absolutely sure of your decision to have surgery. Therefore, once you have agreed to progress, a NON-REFUNDABLE DEPOSIT of $500 is payable when booking. This is deducted from the final surgical fee, which you will be informed of, and must consent to, at the time of booking.
We do allow one change of date, so long as you give us 10 working days’ notice, or if you are ill within 48 hours of the surgical appointment. Please note that a sick certificate will be required.
Why is the fee non-refundable?
Please understand that we do block a space on our surgical list for you and should you not proceed with your surgery, we cannot reuse the space and it becomes a wasted appointment that we cannot use for another surgical patient.
Full payment for surgery is due one week prior to the date of your surgery. Failure to pay will result in us cancelling your surgery and forfeiture of your deposit.
EyeHub offers a reduced fee schedule for pensioners. In line with the Closing the Gap initiative, bulk billing is available for individuals of indigenous/Torres Strait Island descent.
