Your appointment includes an in-depth consultation with Dr Moorthy and, where necessary, a series of initial diagnostic assessments.

Medicare rebates may be applicable with a referral from your GP or optometrist. This will be advised at the time of final billing.

Please note the following:

• Please allow up to two hours for your appointment.
• Additional diagnostic assessment charges will be advised in advance of the assessment
• Medicare rebates may be applicable. We will advise at the time of final billing.
• Pension concessions may be applicable. Please ask reception for details.
• We recommend that you are driven to and from our practice if you are undergoing cataract surgery. Please check when booking.

Please note that failure to cancel or amend an appointment will result in a cancellation fee equal to the cost of the consultation.
