At EyeHub, we aim to provide a smooth and transparent experience. These Terms & Conditions explain our policies regarding bookings, deposits, cancellations, and treatment-related information. By using our website or booking an appointment, you agree to the terms below.

PRIVACY POLICY AND CONSENT TO SHARE INFORMATION

As a private-sector healthcare provider, EyeHub is subject to the National Privacy Principles set out in the Privacy Act 1988. These govern how we collect, handle, use, distribute and store personal information collected from our patients at our clinics.

When interacting with other healthcare professionals, we may be required to disclose personal medical information to obtain an accurate diagnosis or treatment options. By signing this form, you agree and consent to these details being shared when you need them.

DISPARAGEMENT CLAUSE AGREEMENT

The Parties agree not to make any harmful, defamatory, or disparaging statements or remarks, whether oral, written, or electronic, about the other Party, including the Medical Practice, its staff, services, or business operations, to any third party.

This restriction does not apply to truthful statements required by law or to statements made in legal or regulatory proceedings. In the event of a breach, the non-breaching Party may seek remedies, including injunctive relief and damages.

 

PAYMENT AGREEMENT

EyeHub offers consultations, diagnostic tests, and surgical care to our patients. You must settle all consultation and treatment fees on the day you attend. All accounts are finalised before you leave.

Before we schedule your surgical procedure, we will provide you with a written quotation outlining all your expected costs. A deposit secures your surgery date. You must pay the remaining balance one week before your surgery. Unpaid balances may delay or result in the cancellation of your procedure.

We wish to emphasise that seeking treatment without the intention to pay is unlawful. It is a criminal offence to obtain services through deception. Attending appointments or agreeing to surgery with the intention of avoiding payment constitutes this offence.

EyeHub uses respectful and lawful steps to manage unpaid accounts. These steps include reminders, structured payment discussions, and external recovery support when needed. Our goal is to ensure that your care is seamless and that our services are accessible and equitable for all patients.
